Please take a few minutes to fill out the enclosed opinion survey. It asks about your stay, the service and treatment you received, and what you think we might need to improve. Your ratings and comments are important and we take each survey seriously. They are reviewed and compiled by a nationally known research company. The scores and comments are used in staff meetings each month to determine how well we are meeting our goals and what we need to improve upon. So tell us what you think. What was especially good about your stay? What we could have done better? It won’t take very long to fill out the form. We have also provided you with a self-addressed envelope.
