Medical records

To receive a copy of your medical records, you must complete and sign an authorization form to release protected health information. Print the form, then fill it out and sign it. Unsigned requests or incomplete forms cannot be processed.

You can fax the authorization form to 715.735.5491 or email it to releaseofinfo@bamc.org.

Your request will be processed and filled within 10 working days. We will either mail to the address specified, or you may pick up your record at the address above between 7 a.m. and 5 p.m. CT, Monday through Thursday and 7 a.m. and 4 p.m. CT on Fridays. If you will be picking up your health records, a photo ID is required. If someone else will be picking up your records, they must have written authorization from you to pick up your records and must also have a photo ID.

There may be a fee charged, depending on the purpose of the request. We will let you know by telephone how much the charge will be once we retrieve your records from our archives. We provide pertinent information free of charge if requested records are to go to other care providers.

Who owns the medical record?

Bay Area Medical Center (BAMC) owns the physical medical record. The patient owns the information.

How long do you keep the medical record?

Adult medical records are kept for 10 years. Records of patients under the age of 18 at time of treatment are kept 10 years past the date of the last visit or when the patient turns 21 (whichever is greater).

What are your hours of operation?

We are open Monday through Thursday, 7 a.m. to 5 p.m. CT, and on Fridays, 7 a.m. to 4 p.m. CT. We are closed on major holidays.

Is there a charge for copies?

Yes. Patients are charged a $3.20 copy fee and 28 cents per page. Important: There is no charge for records used for continuity of patient care, such as records going to another medical facility or doctor's office.

When can I expect to receive my copy?

Because of the large number of requests we receive daily, it may take up to 10 work days to fulfill your request.

Can you fax copies to my home or office?

No. We cannot fax medical records to your home or office due to confidentiality issues.

Can you send my medical records to my other healthcare providers?

Yes. This type of request is our 1st priority.

Can my spouse request and receive copies of my medical record?

No. He or she may not receive copies of your medical record without a consent form signed by you authorizing BAMC to release copies to him or her.

May I obtain a copy of my Diagnostic Images and reports from you?

Yes.

How do I request a copy of my birth certificate or my child's birth certificate?

BAMC does not keep birth certificates for patients born at the medical center. You may request copies from the Marinette County Register of Deeds at 715.732.7553 or click here to visit its website.

How may I request copies from my last doctor visit?

Contact your physician's office for assistance. We do not keep private physicians' office records.

Can you explain charges on my bill?

No. Please contact our customer service department at 715.735.4203. Their office hours are Monday through Friday, 8 a.m. to 4:30 p.m. CT.
